Bethel Church's insurance is only secondary insurance. If you have medical insurance, your carrier will be billed for medical charges in the case of illness or injury while you, your son or daughter is on a church-related activity.)

I (We) the undersigned parent(s), relative, or legal guardian of a minor, do hereby authorize the Sponsor, Bethel Church or its authorized representative to provide consent to treatment of our child for X-ray examination, anesthetic, medical, dental, or surgical diagnosis, or treatment, or hospital care, which is deemed advisable by a licensed dentist, physician or surgeon whether such a diagnosis or treatment is rendered at the office of the dentist, physician or surgeon or at a hospital facility. It is understood that this authorization is given in advance of any specific diagnosis, treatment or hospital care being required but is given to provide specific consent to any and all such diagnosis, treatment or hospital care which the dentist, physician or surgeon in the exercise of his/her best judgment deem medically necessary or advisable for the health and safety of the patient. This Authorization is given pursuant to California Family Code §6910. This Authorization shall remain effective until revoked in writing. Effective at time of signature.

I acknowledge that participation in the activity described above involves risk to the participant (and to the participant’s parents and/or guardians, if the participant is a minor), and may result in various types of injury including, but not limited to, the following: sickness, bodily injury, death, emotional injury, personal injury, property damage, and financial damage. In consideration for the opportunity to participate in the activity described above (the “Activity”), the participant (or parent/guardian if the participant is a minor) acknowledges and accepts the risks of injury associated with participation in and transportation to and from the Activity. The participant (or parent/guardian) accepts personal financial responsibility for any injury or other loss sustained during the Activity or during transportation to and from the activity, as well as for any medical treatment rendered to the participant that is authorized by the sponsor or its agents, employees, volunteers, or any other representatives (collectively referred to as the “Sponsor”). Further, the participant (or parent/guardian) releases and promises to indemnify, defend, and hold harmless the Sponsor for any injury arising directly or indirectly out of the described Activity or transportation to and from the Activity, whether such injury arises out of the negligence of the Sponsor, the participant, or otherwise.

I understand that I am expected to be on campus when my child is participating in Bethel Kids Ministries program(s). I will pick up my child(ren)/student within 10 minutes of class or service dismissing, unless Bethel Kids or Bethel Student Ministry has communicated in writing a different arrangement. *Please note: Children in Nursery through 5th grade must be picked up by an individual who is 18 years or older. Individuals under the age of 18 will not be permitted to pick them up. Also, for Bethel Kids, you and/or the authorized person that will be picking up your child, will need to have your family's security label to be able to enter the Kids Area. For students in 6th through 12th grade, I give permission for my child to meet up with me.

If a dispute over this agreement or any claim for damages arises, I agree to resolve the matter through a mutually acceptable
alternative dispute resolution process. If I cannot agree with the church upon such a process, the dispute will be submitted to a three member
arbitration panel of the American Arbitration Association for final resolution.
